Diabetes and Endocrine Associates of the Treasure Coast, LLC commits to providing comprehensive and long term care for patients with diabetes and endocrine disorders. Our multi-diverse staff of physicians and support personnel enables us to provide quality care with confidence to our patient population.
In order to make your visit a pleasant one, we’ve summarized a few essential pieces of information so you will know what to expect from us.
Your First Appointment
We require medical records from your referring physician prior to making your appointment. This allows our physicians to become familiar with your prior care and health conditions and determine your necessity for specialty care. You can obtain the Release of Medical Records to Diabetes and Endocrine here. Complete the form and bring it to your current physician.
Please arrive at least 15 minutes prior to your appointment. We ask that you download our forms and complete them at home and bring them to your appointment to expedite the check in process. If you do not have the ability to print, please arrive 20 minutes early in order to complete them in office. Make sure to bring any insurance cards and referral forms with you so that we may copy them.
We are a “NO FRAGRANCE” office and ask you to please refrain from wearing any cologne, perfume, or scented body lotions.
We ask that you arrive 15 minutes before your scheduled appointment time. You should be in the exam room waiting and ready for your physician at your scheduled appointment time. If you arrive more than 10 minutes late, you may be asked to reschedule your appointment.
We are very aware that patients dislike waiting long periods of time in a doctor's office for medical visits. Our physician’s strive to keep on time with their schedule and that is why we ask you to come in early before your appointment. While we try very hard to stay on schedule, sometimes delays can occur. Patients who are sicker or more complicated than we expected are often the reason and we ask for your patience and compassion. If you have waited in our waiting room more than 20 minutes please inform our front office staff.
Diabetic patients are encouraged to bring their blood sugar records to all office visits. If a diary is unavailable but you have a One Touch, Accu-check, Bayer or Freestyle meter, you may download the information at the office. Please arrive an additional 15 minutes early to download.
Patients are always encouraged to bring a record of the medications they take or the medications themselves.
Lab Tests, Radiology and Ultrasound
After the first appointment, we usually order a variety of blood and urine tests. Please complete the tests at least two weeks before your next appointment so that our staff and physicians will have the test results in hand when you return. We have an on site laboratory within our practice for your convenience. Should your insurance plan require a specific plan laboratory such as Quest, we can still draw your blood in office and send it out so you don’t have to make the trip.
We recommend you always check with your insurance first to make sure any test you have done will be covered whether in our office our referred out. Our office is not responsible for cost if you go to a facility that is not covered by your insurance.
Insurance & Billing
We participate in a number of health insurance plans. Each plan has very specific referral and co-payment requirements. It is your responsibility to know what your insurance requires and it is up to you to fulfill those requirements. Your insurance company, not our staff, can answer your insurance plan specific questions.
You may view our Financial Policy online or obtain a copy from our office. Your signature on our registration form and financial policy acknowledges that you will comply with our policy.
For billing concerns, our Patient Accounts Department is open Monday through Thursday 7 a.m. to 5 p.m., Friday 7 a.m. – 12 p.m. Call 772-774-4330 or 772-299-3003 and select option 4 and a customer service representative will gladly assist you with any concerns you may have.
Cancellation & Rescheduling
Because we are a specialty practice, we usually are scheduled out two to three months in advance depending upon the time of year it is. We ask a minimum of 24 hours notice for cancellations or rescheduling in order to be able to fill your appointment spot with another patient.
Appointment Verification
We utilize the Phone Tree messaging system to notify you of upcoming appointments. You will receive an automated phone call from our office the day before your appointment reminding you of your visit. Please listen carefully and follow the prompts to confirm, cancel, or reschedule your appointment.
